These policies help ensure a smooth, efficient, and high-quality patient experience. Please review them before your appointment and contact our office with any questions.
We are committed to helping make your care experience as smooth as possible. Patients are responsible for understanding their insurance coverage and benefits, including co-pays, co-insurance, deductibles, and any remaining balances.
Before your first appointment, please contact your insurance company to confirm that our practice is an in-network provider and that your primary care provider (PCP) information has been updated if required by your insurance plan.
If your insurance coverage changes, please notify our office before your next appointment so we can verify your new plan and ensure continued participation with your insurance network.
If your insurance plan requires a referral, please allow 1–3 business days for processing and approval. If a referral issue arises or a referral is denied, our office will contact you to discuss next steps and available options.
We understand that schedules can change and unexpected situations may arise.
Please contact our office at least 24 hours before your scheduled appointment if you need to cancel or reschedule.
As a courtesy, appointment reminders may be sent by phone call or text message using the contact information you provide. However, patients remain responsible for keeping track of their scheduled appointments, and the practice cannot be responsible for missed appointments due to unsuccessful reminder attempts.
Patients who do not attend a scheduled appointment or cancel with less than 24 hours' notice will be charged a $25.00 fee for each missed or late-cancelled appointment, including new patient consultations and follow-up visits.
This fee may be waived for emergencies or illness at the provider's discretion.
Repeated no-shows or excessive late cancellations may result in discharge from the practice at the provider's discretion.
Failure to confirm a scheduled appointment will result in cancellation. A $25.00 fee will be applied to the patient's account and must be paid before another appointment can be scheduled.
Prescription refill requests should be made during scheduled appointments whenever possible.
If a refill is needed before the next appointment, patients should contact the office at least 4–5 business days before their medication runs out to allow adequate processing time.
Patients prescribed controlled substances will be evaluated on an individual basis.
If a controlled substance is determined to be medically appropriate, a signed Controlled Substance Agreement is required for continued treatment.
The agreement must be renewed annually, and regular follow-up appointments are required to continue receiving prescription refills.
