HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Internal Medicine MD is required by law to maintain the privacy of your protected health information ("PHI"), provide you with this notice describing our legal duties and privacy practices, and abide by the terms currently in effect.

How we may use and disclose your health information

• Treatment — sharing information with other providers involved in your care, such as specialists, labs, or hospitals.
• Payment — using your information to bill and collect payment from you, your insurance company, or a third party.
• Healthcare operations — internal activities such as quality improvement, staff training, and practice management.
• As required by law — including public health reporting, health oversight activities, and responses to court orders.

Other uses and disclosures not described in this notice will be made only with your written authorization, which you may revoke at any time.

Your rights regarding your health information

• Right to access — request a copy of your medical and billing records.
• Right to amend — request a correction if you believe information is incorrect or incomplete.
• Right to an accounting of disclosures — request a list of certain disclosures we've made of your PHI.
• Right to request restrictions — ask us to limit how we use or disclose your information (we are not required to agree, except in certain cases).
• Right to confidential communications — request that we contact you in a specific way or at a specific location.
• Right to a paper copy — request a paper copy of this notice at any time, even if you agreed to receive it electronically.
• Right to file a complaint — with our Privacy Officer or with the U.S. Department of Health and Human Services, Office for Civil Rights, without fear of retaliation.

Our responsibilities

We are required by law to maintain the privacy of your PHI, provide you with this notice, and notify you if a breach occurs that compromises your PHI. We will not use or share your information other than as described here unless you authorize us to do so in writing.

Changes to this notice

We reserve the right to change this notice and to make the revised notice effective for PHI we already have as well as any information we receive in the future. The current notice will always be available on this website and posted at our office locations.
