You can book a visit at our Hudson or Trinity office by phone or by sending the appointment request form on this page. Calling the office is the fastest way to schedule. If you prefer, fill out the form and our team will call you back to confirm a time that works for you. The form is for scheduling only, so please do not include symptoms or other medical details. For urgent needs, call the office directly.
Call 911 for emergencies such as chest pain, trouble breathing, or signs of a stroke.
Both offices share one team of providers and one chart, so you can choose the location that is most convenient for you. You can also ask about telehealth visits for follow up care.
Before your first visit
A little preparation helps your first appointment go smoothly and gives your provider a full picture of your health. Please bring:
- A photo ID
- Your insurance card, including any secondary or supplement cards
- A list of your current medications, including doses, vitamins, and supplements (or bring the bottles)
- Medical records you have, such as recent lab results, hospital discharge papers, or notes from specialists
Please arrive early enough to complete new patient paperwork. Before your visit, you can check that your plan is listed on our insurance page and call to verify coverage. Moving your care from another office? Read our tips on changing primary care doctors and transferring records.
